No, you're right! It's definitely well-written fic that does it for me, and there tends to be a heck of a lot less ER fic around, let alone well-written ER fic. It's much easier to get that quick emotional fix from a first-time fic—because, as you say, there are certain markers that can induce an almost Pavlovian response in a reader (or, y'know, me ;)), whereas it's soooo much harder to get that same oomph from an ER story.
Oddly, the one fandom I can think of that seems to have loads and loads of it (and most of it astoundingly well-written) is Generation Kill. But I'm probably remembering this because I was all, "Yesyes, this is lovely and all, but I want Brad and Nate kissing messily for the first time in a bunker, dammit!" Heh.
